If you file Form I-485, Application to Register Permanent Residence or Adjust Status, you typically must undergo an immigration medical examination and receive vaccinations against vaccine-preventable diseases. If you are required to undergo an immigration medical examination, please bring Form I-693 to your exam. The government requires this for completion of your immigration physical.
Once completed, the results of your examination will be sent to an Immigration Officer and may also be shared with health departments and other public health or cooperating medical authorities.
To ensure a smooth and efficient appointment, please bring the following items:
- Government issued picture identification, such as an unexpired passport, driver’s license or ID. School IDs are not acceptable.
- For applicants under 14 years of age, USCIS will accept other proof of identity that shows name, date and place of birth, parents' full names, and any other identifying information about the applicant. Acceptable documents include birth certificates (with translations, if necessary) or affidavits.
- Your medical records for any chronic or serious medical problem(s).
- Your immunization records, including childhood records, if available (with translation, if necessary) or an affidavit.
- Alien Registration number (A-Number) if you have one.
- The most recent copy of the I-693 medical form if you have one. We can provide this, if needed.
- Translator, if needed, must be present to sign forms.
If medical records are not in English please have them translated before your appointment.
